Class Reschedule & Transfer Forms

We’re sorry you weren’t able to attend your scheduled class. According to our reschedule and refund policy, you are allowed to reschedule once. Failure to attend the second class date will result in the forfeiture of any registration fees paid. You have 12 months from your original class date to reschedule and attend your new class. If you do not reschedule your class within 12 months, all registration fees will be forfeited. Complete the “Reschedule Form” below.

You also have the option to transfer your registration to another interested participant by completing the “Transfer Form” below.

Check your original class registration receipt or confirmation email to make sure you are completing the correct reschedule form.
